Meet our school health team:
  • ​Courtney White​, RN, BSN is the full-time nurse that oversees the three schools for regular and special education. Courtney began her nursing career in the hospital setting, where she most recently specialized in gastroenterology nursing. She is dedicated to furthering her education in school nursing to provide excellent care to the community.
  • Kelsi Pickard​, RN, BSN, is a full-time nurse overseeing the middle and high schools.  She began her nursing career in Texas and has worked in multiple places across the U.S. Most recently, she was an ICU nurse in Kankakee.  Kelsi is a graduate of Reed-Custer High School and is excited to be back helping students in her home town.  
  • Alyssa Mota, CNA, is a full-time nurse assistant at elementary, middle and high school. She began as an EMT-B  at Ingalls hospital, then move to a fast paced environment  at Silver Cross Hospital on the  observation and cardiac units. She is looking forward to using her skills in this new roll with Reed-Custer!

  • Donna Ferrari, RN, provides care to SOWIC and regular education students at Reed-Custer Elementary School.
  • Tara Phelan, RN, is a substitute nurse at Reed-Custer Elementary School for both general education and SOWIC students.  

Illinois School Health Mandated Requirements

The below mandated health requirements should be turned in on the first day of school.  If required School Physical with up to date vaccines are not turned in by September 1st, your student will be excluded from school until required forms are received.

·        Vision Exam

DTaP- 4 doses

2nd Grade

6th Grade

·        School Physical with up to date vaccines

All Kindergarten vaccines, plus

9th Grade

No new vaccines required, must be up to date

12th Grade

·        Proof of Vaccine

MCV (meningitis- MCV4 or MenACWY)- 2 doses (1 given after age 16)

New Students (have been in IL before)

Most recent mandate year’s requirements (new 7th grader would need to provide their 6th grade physical and dental) Usually, we get these records from previous school, but, if not, the parent must provide it. (due within 30 days of start date)

 

New Students (have never been an IL student before)

·        Illinois School Physical with up to date vaccines (due within 30 days of start date)

Health Forms
Below find all health forms that may be needed for your student during their time at Reed-Custer School District. 
 
School Physical
This document is due before September 1st each school year. It must be completed within one year of the first day of school and contain a full and up to date vaccine history to be considered valid. The religious exemption document is to be completed if your student is not up to date on their vaccinations. 
 
Dental Exam
This document is due before May 15th of each school year. It must have been completed within 18 months of the due date to be considered valid. Please complete a waiver if your student will be unable to complete the dental examination. 
 
Vision Exam
This document is due before October 15th of each school year. It must have been completed within 1 year of the due date to be considered valid. Please complete a waiver if your student will be unable to complete the vision examination. 

Action Plans

If your student has any of the conditions below, please have their doctor complete the appropriate action plan. A new action plan is needed every school year and anytime changes are made. Whenever possible, this form should be turned in before the start of school in order to ensure that necessary staff members are aware of all pertinent information. If your doctor prefers to use their own form, that will be accepted as well.

Medication Administration

If your student will need to take a medication while at school, please have their doctor fill out the medication order form. If your student will carry their inhaler and/or Epi-Pen with them during the day while at school please fill out the medication self-administration form in addition to the corresponding action plan.

Athletics Information

Below is the IHSA/IESA Sports Physical. This can be used for participation in sports only. This physical is NOT VALID for health mandates.
